Social Care Network What is this program
A community support network designed to help Medicaid members in Monroe Country and the Finger Lakes Region access more than just medical care. WE provide assistance with housing, food, nutrition, transportation, and other essential services the support your health and well-being.

Who is eligible
Any qualified Medicaid member living in Monroe County or the Finger Lakes Region. To qualify for assistance, members must complete a Health-Related Social Needs Screening to identify their needs for support such as housing, food, nutrition, and transportation.

Services we offer:
Housing Support: Assistanct with rent, temportary housing, utility setup, home modifications, housing transitions, and basic payment support.
Food & Nutrition: Access to prescriptions, food pantries, fresh produce, and nutrition resources.
Transportation: Help with medical appointments and related social services.
